The effectiveness of the use of multimodal anesthesia in patientsoperated for neoplasms of lung

Abstract

We examined and treated 74 patients (59 men and 15 women) aged from 46 to 60 years with malignant neoplasms of the lung. 42 patients were operated on under multimodal anesthesia with epidural blockade, and 32 patients- under inhalation and intravenous anesthesia with artificial lung ventilation. In the preoperative, intraoperative and earlypostoperative period in blood plasma the concentration of adrenaline, noradrenaline, dophamine, cortisol, insulin andglucose, and parameters of acid-base composition in arterial blood were studied. Found that the most important pathogenicfactor that causes stress reactions in patients operated for malignant neoplasms of lungs in the conditions of stationaryof pain relief, is a marked activation of sympathoadrenal system. This is manifested by metabolic disorders, nociceptionand oxygen balance in the body. The operations performed in the multimodal anesthesia, showed minimum change in thebasic parameters of homeostasis, were short-term, compensated and reversible. Pain syndrome after operation and in earlypostoperative period was absent or less expressed.
